Sleep-away camps can cost an average of $500 or more per week. However, thanks to our generous donors, you only pay $25 per child, per week, for the same quality experience.

The following periods are available:

Music & Creative Arts Camp** -    Saturday, July 3 - Saturday, July 10, 2010 (8 days & 7 nights)   
** Reserved ONLY for children involved in Salvation Army musical programs throughout the year.

Adventure Camp- ++                    Tuesday, July 13 - Tuesday, July 20, 2010 (CLOSED)
Mega Sports Camp - ++                 Friday, July 23 - Friday, July 30, 2010 (CLOSED)  
++Transportation provided from 120 West 14th Street - New York, NY.

Corps Program Camp -                  Monday, August 2 - Monday, August 9, 2010 (8 days & 7 nights)
Transportation is not provided from 120 West 14th Street - New York, NY.                 

Numbers 1-8 listed below must be completed for each child.  No child will be considered registered until ALL documents are received by our office. For your own records, we ask that you make a copy of all completed forms before they are submitted to our office. 1.  Registration Checklist

We encourage you to use this form and attach it to your child(ren)'s packet before submitting their application.  Incompleted applications will not be processed and will only delay your child's registration.  Registration is not finalized until all documentation is completed and submitted.

2.  Camper Health History

Please Note:  No child will be registered without a properly completed medical form. 

Please make sure that all emergency contact information is completely filled in, especially names and telephone numbers (including area codes).   Any food or physical restrictions, recent injuries and/or medical issues must be clearly noted on the form.  Any missing information will delay this process.

3.  Summer Food Service
FYI:  Non-profit camps are entitled to reimbursement for a percentage of the cost of meals for all campers whose income falls within stated Federal and State guidelines under the Summer Food Services Program for Children.  Therefore...

4.  Camper Questionnaire & Behavior Contract

Camper Questionnaire:  This form is to help your child's counselor get acquainted with him/her prior to arrival.

Behavior Contract:  This form states Star Lake Camp's standards for camper behavior, policies and procedures for handling disruptive behavior.

6.  Camper Application

 Applications can no longer be submitted on-line.  All forms must be printed out and mailed in or hand delivered.  For your convenience, you may type in the information (on-line) and then print it out or you can simply just print out the application and handwrite your information.

7. Clubs/Electives

You can now select (in order of preference) your club choices for Adventure Camp & Mega Sports Camp.  Classes will fill on a first come-first served basis.  If you do not select any activity, you will be automatically placed in one.

8.  Challenge By Choice Program Waiver

A waiver form must be completed for each child if he/she is interested in participating in any of our adventure activities including but not limited to our Ropes Course and Climbing Tower.

9.  Music & Creative Arts Camp - Major Track Classes

Children who are involved in a music program throughout the year in a Salvation Army Corp must complete this sheet if they are interested in attending Music & Creative Arts Camp.  Please thoroughly read the descriptions for each class as some require a specific instrument to be brought to camp.
